In order to participate in our program, we require our patients to get educated about weight loss surgery procedures and gain an understanding about the journey on which they are about to embark.
There are two options for meeting this requirement so that you can make an informed decision in choosing the procedure that is best for YOU!
Option #1
Watch our Online Educational Presentation and then complete our Interactive Test.
Passing the Bariatric Patient Comprehensive Test with a minimum score of 75% is a requirement for patients who choose to view the Online Educational Presentation as the method of meeting the program's education requirement. It will measure your understanding of our Bariatric Program and will help us determine your qualifications for surgery. PLEASE PRINT OUT THE TEST BEFORE CLICKING ON THE "EMAIL" BUTTON, IN CASE OF INTERNET FAILURE. PLEASE CALL THE BARIATRIC COORDINATOR AT 818-676-4141 TO ENSURE YOUR TEST WAS RECEIVED.
